Introduction

Sanos Clinic collects health information to help find new treatments.

By creating a profile, you consent to Sanos Clinic contacting you regarding possible participation in a survey or clinical study.

Any profile may be deleted by Sanos Clinic at any time upon your request.

By registering for the database, you agree to receive emails, SMS and phone calls from Sanos Clinic for service notifications regarding meeting times, possible fasting visits or other study-relevant reminders when participating in a study.

If you no longer wish to receive emails, SMS or phone calls, you can unsubscribe from the service at any time.

Questionnaire surveys and clinical studies are not mandatory, and you can refuse to participate at any time. Storage and other processing of your information in our database is encrypted.

By creating a profile, you consent to Sanos Clinic collecting a number of personal data from you for the purpose of processing them to pursue the purposes mentioned above, i.e. to contact you regarding possible participation in a questionnaire survey or in a clinical study. The personal data will include the following:

  • Contact information
  • Age
  • Residence
  • Weight
  • Height
  • Medical history
  • Medicine
  • Dietary habits, including alcohol and tobacco consumption

The detailed terms of this consent and your rights in connection with giving consent are set out below.

By submitting this form, you agree to all terms of consent. Any profile can be deleted at any time. If your profile is deleted, all data will be permanently deleted.

If there are any changes to our terms, you will receive an email about this. You will then have the option to either accept the changes or delete your profile.

Terms of consent and notification of collection of personal data

This document contains information for you about how the personal data we receive from you is processed.

3. The purposes and legal basis for the processing of your personal data

We process your personal data for the following purposes:

Sanos Clinic collects your personal data to determine whether a notification of possible participation in a questionnaire survey or a clinical study should be sent to you.
It is noted that the terms of any participation in a clinical study or a questionnaire survey, including consent to the collection of your personal data for this purpose, will be stated in a separate consent form, which you will receive if and when you wish to participate.
If you are considered interesting in relation to possible participation, we will contact you by e-mail, SMS or phone call.

The legal basis for our processing of your personal data is the consent you have given in connection with your creation of a profile in the Sanos Clinic database.

4. Categories of personal data

We only process the categories of personal data about you that are stated in the introduction to this consent statement.

5. Recipients or categories of recipients

We do not disclose or transfer your personal information to any companies or individuals outside our organization.

6. Where your personal data comes from

The personal information about you that is processed originates solely from you and is thus obtained when you create a profile in Sanos Clinic’s database.

7. Storage of your personal data

We will store your personal data for as long as your profile is registered in our database. If you delete your profile, your personal data will also be deleted.

8. The right to withdraw consent

You have the right to withdraw your consent at any time. You can do this by contacting us using the contact details provided above in point 1.

If you choose to withdraw your consent, it will not affect the lawfulness of our processing of your personal data based on your previously given consent up to the time of withdrawal. Therefore, if you withdraw your consent, it will only take effect from that time.

9. Your rights

Under the General Data Protection Regulation, you have a number of rights in relation to our processing of information about you.

Right to see information (right of access)
You have the right to gain access to the information we process about you, as well as a number of additional information.

Right to rectification (correction)
You have the right to have incorrect information about yourself corrected.

Right to erasure
In special cases, you have the right to have information about you deleted before the time of our regular general deletion occurs.

Right to restriction of processing
You have the right to restrict the processing of your personal data in certain cases. If you have the right to restrict processing, we may only process the data – with the exception of storage – with your consent, or for the establishment, exercise or defence of legal claims, or to protect a person or important public interests.

Right to object
In certain cases, you have the right to object to our or lawful processing of your personal data.

You can also object to the processing of your information for direct marketing.

Right to transmit information (data portability)
In certain cases, you have the right to receive your personal data in a structured, commonly used and machine-readable format and to have this personal data transferred from one data controller to another without hindrance.
